Description
To primarily support the hospital discharge process by safely and sustainably supporting patients back home and back to independence in the community – avoiding re-admissions. Alongside this, to support residents in the community, to prevent, avoid, reduce and delay any need for admissions into the health care system; for those who may require some low level, time-limited support to regain independence. The service will provide seven-day support, working closely with health and social care to deliver integrated and co-ordinated care to adult residents to: • facilitate early, safe and sustainable discharge from Newham University Hospital (‘NUH’); • reduce delayed transfers of care in NUH by supporting other partners (e.g. the assistive technology service, the integrated community equipment service, the enablement service, etc.) to facilitate discharge; • reduce avoidable re-admissions over a 90-day period; • prevent avoidable attendance.
Total Quantity or Scope
The service shall offer up to six weeks of support — incorporating a number of practical services including: — healthy home checks (please refer to paragraph 4.4.5 of this specification); — practical low-level tasks (please refer to paragraph 4.4.6 of this specification); — handyperson service (please refer to paragraph 4.4.7 of this specification); — signposting to other services. That enable customers to regain confidence, motivation and skills to self-manage back to their baseline and encourages customers to engage with the local community to improve socialisation and wellbeing. The service shall work closely with partners to deliver an integrated whole system approach to achieving the outcomes for customer and the overall health and social care system; applying a ‘can-do, will-do’ approach to delivery and ongoing service development. The provider will deliver a responsive service to support the discharge of patients from NUH and work with community partners to support prevention of any unplanned admissions e.g. working with East London Foundation Trust (ELFT) to support residents who have fallen in the community. The service shall support partners to deliver services that facilitate hospital discharge (e.g. the integrated community equipment service in setting up a micro-environment, the assistive technology service providing access for telecare installation, etc.).
