Description
Provision of a Homeless Health Peer Advocacy Service (HHPA) for residents within the WLCCG footprint, to deliver specialist one-to-one engagements to enable people experiencing homelessness to register with GPs, make and attend health appointments and have social needs addressed. In addition to providing practical support, such as travel fares and accompaniment to appointments, peers will focus on building the skills and confidence to enable clients to continue accessing health services independently. The Service will include: • Peer Advocates: peer advocates with lived experience of homelessness. Shared experience with clients to enable them to build the trusting relationships required to get people to appointments. Peers to acts as role models - proving you can improve your health, gain independence and move on from homelessness. • Advocacy Support: Peers to provide one-to-one support to enable homeless people to make, attend and maximise healthcare appointments. Working with homelessness services across the area to ensure appropriate referrals for clients with urgent but non-emergency health needs - engaging people with specialist and mainstream primary care and ensuring people are registered with GPs, dentists and opticians. • Health Promotion: Peers to run health promotion events in homelessness services across the area

Other Information
Modification of an existing contract to effect a 6 month extension to current contractual arrangements, ensuring continuation of an essential service. This extension will afford the newly formed NHS North West London CCG time to review and develop substantive future commissioning requirements. The CCG has assessed it to be reasonable, proportionate and lawful to modify the existing contract in this manner. The existing contract 2-year contract commenced 1st April 2019. The 6 month extension will run 1st April 2021 until 30th September 2021.
