Description
The Home from Hospital service provides short-term home-based support to individuals being discharged home following a stay in hospital. This is a ‘non personal care’ service and does not require CQC registration and therefore the provision of this service is an opportunity for the care and voluntary and community sectors alike. It aims to support customers to regain their confidence and independence with a combination of both practical (hands on), emotional and social support, following the principle of a planned and phased reduction in support. In providing such support to the customer, Wiltshire’s care services are also supported by releasing care capacity to be used for personal care, with the ‘Home from Hospital’ service supporting the non-care elements. Alongside this, the service also offers transport home from hospital, if no other alternative is available. The service is also expected to telephone pathway zero patients 48 hours post discharge to ensure that they do not have any outstanding needs. This element requires the signposting of people to appropriate services, both formal and informal. The service operates over 7 days for those aged 18 years and over.
Renewal Options
The awarded contract can be extended by a period of up to two years from it's initial expiry date of the 30/06/2025.
