Description

The service will be provided at the Princess Royal University Hospital Trust (PRUH) in Farnborough. The service will ensure that a hospital patient's voice and wishes are heard regarding their treatment, at meetings and when planning their future care when they are due for discharge. This service will provide an advocate to visit the patient, who will listen and understand their issues. They convey these to the relevant agencies to ensure that the client's voice is heard. Referrals are taken from Hospital Social Services Teams, hospital staff, patient themselves (self-referral), patients' relatives and/or careers, Bromley CCG managed services and other hospital-based services. In addition, referrals can be accepted from the wider community, local GPs, social services and other support agencies. The service should help facilitate a safe hospital discharge, with patient's wishes to be taken into consideration, within the appropriate provision available to meet their needs. The service will meet the needs of those patients over the age of 18 (registered with a Bromley GP) who require their voice to be heard whilst an inpatient in the PRUH. The service will be made available from Monday to Sunday between 9.00am to 5.00pm. The Contract is going to be for 3 year. The Contract is expected to commence on 01 July 2022. The expiry date of the contract is expected to be on 30 June 2025.

This is a call for competition. In the event the NHS SEL CCG receives only one expression of interest by the above deadline, the NHS SEL CCG reserves the right to enter into direct negotiations with that supplier to award a contract. The deadline for the expression of interest is 14:00 on 17 June 2022. Interested organisations must download the expression of interest questionnaire. A duly completed expression of interest questionnaire (PRJ - 1154 EOI Bid Response Document) must be submitted via the messaging facility on this project on the Pro contract. Aims and objectives of service The principle of advocacy is to offer support and to speak on behalf of people who feel unable to do so themselves. Service users are encouraged to help themselves where possible, instilling the value of independence whilst receiving advocacy support. To ensure that a hospital patient's voice is heard regarding their treatment, at meetings and their future care when they are due for discharge. The patient will have had their wishes heard and taken into account regarding their future care on discharge. 3.2 Service description/care pathway An advocate visits the patient / service user and listens and understands their issues. They convey these to the relevant agencies to ensure that the client's voice is heard Days/Hours of operation Monday to Sunday . Referral criteria & sources Service users must be in hospital awaiting a discharge and referrals are taken from hospital staff, social services, service user's relatives and carers and self-referral. Referral route Hospital staff, social services, service user's relatives and career's and self - referral. Service users' details must be logged with the service provider and comply to NHS Information Governance and Data Protection minimum standards. Response time & detail and prioritisation Potential service users are contacted within 1 working day and a visit is arranged. The Expression of Interest is only open to SME organisations that are based in South East London.
