Description

NHS North West London (NHS NW London) requires immediate provision of Severe Mental Illness (SMI) Outreach Engagement Services in the borough of Hillingdon. NHS NW London currently has an arrangement in place with MIND in Hillingdon to deliver the above services. This notice is to be transparent about the intention to award a further contract (NHS Standard contract-short form) to MIND in Hillingdon from 1st July 2023 until 31st March 2024 (estimated value of £53,008). Success of the service is linked to close partnership working and integration with the wider pathway of services in Hillingdon. As such close partnership working and a multi-disciplinary approach will be needed, NHS NW London believes that MIND in Hillingdon is best placed to deliver the service. Aims and objectives of the service: -Support Hillingdon GPs to put in place a local search to keep track of SMI physical health check progress and to share that information with our local SL providers, so we can identify people who need support encouragement to have an SMI health check, specifically those from black and minority ethnic groups who need more intensive support for them to engage. -Work with practices and support the use of GP receptionist staff to systematically call SMI patients and invite them in - targeting 75% of the practice list to come in for their physical health checks. -Educate all local Supported Living and Residential providers about SMI health checks and the importance of supporting service users to attend GP practices for the checks. -Work with practices and PCN's to develop a model of SMI check that is person centred and delivered from community settings that supports people with SMI Greater access to and take up of other services. Key Service Outcomes: -Provide educational briefings to supported housing staff and any relevant supporting staff about the importance of SMI checks and how to reassure Patients who have concerns. -Provide performance measurement reports on numbers of SMI's being completed. -Provide information on those patients who maybe refusing SMI checks and noting any areas of concern, making safeguarding referrals etc. -Create and co-ordinate all promotional and publicity materials in relation to SM. -Contribute intelligence to key bodies to assist in future service design.
