Description

This contract will cover residents of Bracknell Forest who require additional support to return home once discharged from hospital. The service will liaise with key stakeholders to receive and action referrals. A questionnaire is located, as an attachment, on the South East Business Portal https://sebp.due-north.com/ advert. Please complete the questionnaire and return it via the messaging facility on the portal. Projected spend is between £70,000.00 and £80,000.00 per annum with a provisional contract duration of 4 years (2+1+1). People will be supported for a short period of time, with visits and telephone calls to help with emotional needs and small practical tasks. The service is not expected to provide personal care. Key aims for this service are to: • Support patients being discharged from hospital • Enable people to live independently in their own home • Improve patient experience • Reduce hospital re-admission • Assist with hospital discharge • Reduce hospital readmission rates • Supporting patients to be fully sighted on additional community organisations where appropriate The Home from Hospital service will work closely with the Adult Social Care team, OT's and ward staff based in hospitals to help with the timely discharge and unnecessary re-admittance of patients. The service will also work closely with the Intermediate Care Teams with referrals coming through from Intermediate Care as well. Projected spend is between £70,000.00 and £80,000.00 per annum with a provisional contract duration of 4 years (2+1+1).

The Council is currently undertaking market engagement to help inform the specification for the new contract which will be going out to tender late Autumn.
