Description

The service will provide timely reactive care, anticipating disease exacerbation and manage acute illness based on the providers in depth understanding of long-term conditions as part of a person-centered approach. The provider will ensure the service provides high levels of communication, problem-solving and decision-making skills that will ensure risk can be managed appropriately. The provider will ensure that staff take responsibility for their own decisions, through advanced clinical skills including non-medical prescribing and physical assessment to support effective clinical decision making in relation to the wellbeing of care home residents.

Total Quantity or Scope

The service will provide timely reactive care, anticipating disease exacerbation and manage acute illness based on the providers in depth understanding of long-term conditions as part of a person-centered approach. The provider will ensure the service provides high levels of communication, problem-solving and decision-making skills that will ensure risk can be managed appropriately. The provider will ensure that staff take responsibility for their own decisions, through advanced clinical skills including non-medical prescribing and physical assessment to support effective clinical decision making in relation to the wellbeing of care home residents. The relevant authority intends to award a contract to an existing provider under Direct Award C under the Health Care Services (Provider Selection Regime) regulations 2023. The final contract value is still to be finalised as it is dependent on the national NHS planning guidance which is still to be released. As an indication the 2023/24 contract value is £406,182. Contract Start Date: 01 April 2025 Contract End Date: 31 March 2027
