Community health services - assisted discharge and community support service (for those who have been discharged).

Total Quantity or Scope

The notice relates to an assisted discharge and community support service (for those who have been discharged). In the assisted discharge service, the activities are to ensure safer and faster discharge from an acute hospital e.g. delivering TTA medications, ensuring the property is ready for the patient and escorting the patient home for up to 72 hours. The community support service includes confidence building, shopping, escorting to a social/activity group, completing paperwork, and improving the home environment for up to four weeks. This notice is an intention to award a contract to the existing provider following direct award process C.The lifetime value of the contract is £160,000The expiring contract ends 31 March 2025

This is a Provider Selection Regime (PSR) intention to award notice. The awarding of this contract is subject to the Health Care Services (Provider Selection Regime) Regulations 2023. For the avoidance of doubt, the provisions of the Public Contracts Regulations 2015 do not apply to this award. The publication of this notice marks the start of the standstill period. Representations by providers must be made to decision makers by 14 March 2025. This contract has not yet formally been awarded; this notice serves as an intention to award under the PSR.
