Description

The service will be an integrated referral system delivered alongside Durham rehab service supporting service users through community rehab who have been discharged. The service will provide both early intervention services and prevention of readmission. The service will be provided by 2 x 0.8 WTE Mental health wellbeing recovery practitioners and 1 x Activity coordinator to provide a coordinated programme of support and fill resource gaps and provide support to the Rehabilitation team within TEVWS Durham sites. The service is expected to provide a flexible approach to supporting service users with recovering metal ill health to include activities for health and wellbeing and reintegration support.

Total Quantity or Scope

Durham Community Rehab Support Tees Esk and Wear Valleys NHS Foundation Trust are intending to award a contract to an existing provider following direct award process C. The approximate lifetime value of the contract is £400,000 The contract term will be for 36 months from the 1st April with an option to extend for a further 24 months.
