This award is for an urgent complex care package to be put in place to ensure continuity of care for the patient.The initial contract period is for 16 weeks from 10th September 2024 to 31st December 2024. The initial contract value is £70,476.96.The contract includes an option to extend for a further period of 6 months to 30th June 2025 for a value of £114,525.06.
