Description
The Lewisham NHS@Home Service is a healthcare delivery model designed to provide comprehensive and coordinated care for patients outside of traditional hospital settings. It leverages technology, remote monitoring, and a multidisciplinary approach to manage the health and well-being of individuals, particularly those with chronic or complex medical conditions. The objective of NHS@Home is to reduce pressure on the acute healthcare system in Lewisham. This is achieved by 1) avoiding admissions and presentations to the emergency department, and 2) by facilitating supported discharges for patients already in the acute healthcare system. The target population are Lewisham residents, over the age of 18, who would otherwise require hospital level intervention. It is assumed that the majority of patients accessing the service will be within the elderly, frail population.
Total Quantity or Scope
Lewisham NHS@Home Service. The current provider of the Service is meeting the contractual requirements to a satisfactory level and the intention is to award the providers the contract for the period 1st April 2025 to 31st March 2026 (with break clauses at 6 and 9 months), following PSR Direct Award Process C as there is no change to the contract terms. The lifetime value of the contract is £1,187,554, based an annual term.

This is a Provider Selection Regime (PSR) intention to award notice. The awarding of this contract is subject to the Health Care Services (Provider Selection Regime) Regulations 2023. For the avoidance of doubt, the provisions of the Public Contracts Regulations 2015 do not apply to this award. The publication of this notice marks the start of the standstill period. Representations by providers must be made to decision makers by midnight on 2nd April 2025. This contract has not yet formally been awarded; this notice serves as an intention to award under the PSR. Award decision makers: Lewisham Senior Management Team on 18th February 2025. There were no conflict of interest identified in the decision making process. The evaluation was undertaken as follows: 5 key criteria selected (1) Quality and Innovation (2) Value (3) Integration, collaboration and service sustainability (4) Improving access, reducing health inequalities and facilitating choice (5) Social value. Each of these criteria applied an equal weighting. Current performance assessed against a threshold score of 50% of the available score for each key criteria (ie 10%). Potential future performance assessed against a threshold of either 50% of the available score for each criteria (ie 10%) Provider assessed on a Pass/Fail basis against basic selection criteria. All Scored and Pass/Fail criteria are met.
