Support for people and families affected by Alzheimer's and Dementia registered with a GP in Cheshire East Place. Meeting the needs of those who have been identified with a deteriorating condition.

Total Quantity or Scope

NHS Cheshire and Merseyside ICB intend to award a 1 year contract via Direct award C to incumbent provider to deliver Dementia support services across Cheshire East Place. The contract will start on the 16th May 2025, expire on 31st March 2026 and has an annual contract value of £81,108.
