Description

The Services will support Children and Young People with a range of Disabilities, including Children and Young People with physical and learning Disabilities, autism and Challenging Behaviours, and many children have multiple and/or complex needs. The Framework will cover a range of Care and support Services across Staffordshire which are tailored to the specific needs of eligible Children and Young People. The Services will support Children with Disabilities but can also extend to Children in our safeguarding and Edge Of Care services. This is a Light Touch competitive framework with six (6) Lots: Lot 1- Community Access and Socialisation Lot 2- Home Care (Domiciliary Care) Lot 3- Sitting Service Lot 4- Complex/Urgent Domiciliary Care Lot 5- Complex/Urgent Behaviour Support Lot 6 Lifeskills & Independence Programme

Lot Division

1 Community Access and Socialisation

To provide Care to Children and Young People (0-18) (with SEND) and their Families, to aid access to community settings and Socialisation. This support will help Children and Young People learn and acquire new skills and assist them in preparedness for adulthood, as well as to aid Families to stay safe and well together at home, without an escalation of needs. The Service will focus on building resilience, maximising independence of the C/YP, following a strengths-based, restorative approach. Providers will deliver a quality and effective Service personalised to each C/YP's Assessed Need(s).

2 Home Care (Domiciliary Care)

To provide Care to Children and Young People (0-18) (with SEND) and their Families in their home (which is sometimes referred to as Domiciliary Care), to help Children and Young People to be as independent as possible and assist them in preparedness for adulthood. This Care aims to aid Families to stay safe and well together at home, without an escalation of needs and supporting Carers in their caring role. The Service will focus on building resilience, maximising independence, following a strengths-based, restorative approach. Providers will deliver a quality and effective Service personalised to each C/YP's Assessed Need/s.

3 Sitting Service

This Service is to ensure Carers are supported to take break from their caring role and responsibilities for C/YP age 0-18 who have SEND. The Provider must offer a flexible Sitting Service that can be provided during the day, evening, or night for any length of time, as follows: • C/YP can be supported in the home, giving the Carer time to go out if they choose. • C/YP may wish to go out in their local community for the allocated hours to pursue their own interests. Care Staff can accompany Carers and the C/YP on outings to provide the extra support required for the whole Family unit.

4 Complex/Urgent Domiciliary Care

Providers will be expected to deliver Care in an urgent situation/emergency, within the C/YPs home or hospital setting, to ensure the C/YP Social Care Needs are met. Support will be individualised to the C/YP needs, enabling the Family to stay safe and well together, deescalating the risk of the C/YP entering the local authority care system. This Care required could be surrounding complex care needs and life limiting conditions. The Provider may need to work alongside healthcare and other professionals to deliver Care. The Provider must comply with all Relevant Legislation. The Service will be provided by appropriately qualified Staff, in line with the Essential Standards set out by the CQC.

5 Complex/Urgent Behaviour Support

Providers will be expected to deliver Care in an urgent situation/emergency, within the C/YPs home or community, to ensure the C/YP Social Care Needs are met. Support will be individualised to the C/YP needs, enabling the Family to stay safe and well together, deescalating the risk of the C/YP entering the local authority care system. This Care could be surrounding complex needs and Challenging Behaviour related to Learning Disabilities, Autism and/or trauma. The Provider may need to work alongside healthcare and other professionals to deliver Care. The Provider must comply with all Relevant Legislation. The Service will be provided by appropriately qualified Care Staff, in line with the Essential Standards set out by the CQC.

6 Lifeskills & Independence Programme

The Life skills & Independence programme will help Young People with a Special Educational Need or Disability (SEND) to achieve their dreams or goals and to develop Life skills for the future. The Provision of Life skills and Independence support must enable C/YP with SEND to work towards individualised, person-centred Outcomes with appropriate support to access their local community, preparing them effectively for adulthood and equip C/YP with SEND and their Carers with the tools and confidence to actively participate in their local community. The Life Skills and Independence Programme can benefit and provide C/YP with SEND with an opportunity to: • Seek appropriate employment opportunities. • Promote independent living. • Spend time away from their Carers. • Socialise with peers and make new friends. • Learn new skills. • Keep physically active. • Improve their communication skills. • Access activities which support them to develop self-esteem, confidence, and social interaction skills. • Gain independence To qualify as a Lifeskills and Independence Short Break opportunity, any support period must: • Last a minimum of 2 hours and maximum of 4 hours per week; and • Be facilitated outside of the school day - this can include at an evening, weekend or during the school holiday.

Renewal Options

At the Authority's entire discretion, the term may be extended by one 18 month period on the same terms for such period(s) in accordance with the contract.
