Description

The Council wishes to commission a service to provide support to people living with dementia. The service will be provided to people of any age living with a dementia. The successful provider will play a critical part in delivering service to reduce social isolation, improve and maintain their health and wellbeing and empower people to have more independence, autonomy, choice and control in their lives. The service will provide a pathway of support for people living with a dementia from early onset through to advanced end stage dementia. The service provider must provide specialist centre-based provision for people living with a dementia facilitation meaningful and individualised day opportunities.
