Health and Wellbeing Practitioner Supports to Diabetes Young Persons Clinics
Total Quantity or Scope
The Trust are intending to award a contract to an existing provider following direct award process C. The Well Centre will provide GSTT with two Health and Wellbeing Practitioners for outpatient clinic care, to support young person’s diabetes services across GSTT and King’s College Hospital (KCH), as well as Lambeth and Southwark community clinics. The Health and Wellbeing Practitioners will attend the Young Person’s clinic at GSTT/KCH and engage with young people as part of a multi-disciplinary care team. Responsibilities will include one-to-one engagement with young people outside of clinic / activity environment, which may include school visits, home visits or meeting in the community, maintaining the GSTT/KCH diabetes young person’s WhatsApp group and the twitter (X) account, and collecting data on the case load.The contract is for one year and approximate lifetime value of the contract is £125,641.74.
