Description

To support the appropriate discharge of SWYPFT patients who are facing delays due to challenges around their housing in the community e.g. no accommodation or a return to their home may be compromised for a number of reasons. Also supports the tenant to resettle into the community and aid them in finding a more long-term housing solution.

The awarding of this This contract is subject to has been awarded under the Health Care Services (Provider Selection Regime) Regulations 2023. For the avoidance of doubt, the provisions of the Public Contracts Regulations 2015 do not apply to this award. The publication of this notice marks the start of the Standstill Period. Representations by providers must be made to the relevant authority by 24 October 2025.
