Description

NHSE Health and Growth funded project piloting a new approach for caring for people with multiple LTCs. The provider will adopt a new way of working to improve and understand how we can best configure services around the needs of people with multiple LTC's (with a focus on the working age population from September within the cohort, still focusing on our priority PCN areas).
