Community Opportunities - Flexible Purchasing Solution

2. II.1.4 Short Description

The Council is seeking applications from providers who wish to join the Community Opportunities: Flexible Purchasing System (FPS). The intention is to appoint a range of providers with the capability and capacity to deliver Community Opportunities. The Learning Disability/Autism - Community Opportunities is accessed by adults requiring specialist support and who have long term health conditions and/or complex needs (including complex behavioural needs). This could include learning disabilities, sensory impairment, autistic spectrum disorders, multiple physical disability, epilepsy, dementia, and additional mental ill health. Individuals with acquired brain injury may where appropriate be referred to the service. The service promotes people to use community opportunity services as a transition to independent living / independence and to operate with a preventative focus. The initial duration of the model is 5 years but may run beyond this term if it continues to be fit for purpose. Rotherham Metropolitan Borough Council are inviting applicants who wish to deliver Community Opportunities in Rotherham to apply to join the Councils Flexible Purchasing Solution (FPS). The opportunity is open to any provider, including small community groups and micro enterprises, who want to offer creative outcome focused services to people with Autism or Learning disability. The Council has now published the application documentation and notices to the market via YORtender (YORtender ID:53601) and encourages support providers to apply. Organisations can register free of charge and download the application documentation. A market engagement event for potential applicants is being held on: Tuesday 25th March at 1pm at Rotherham Town Hall (John Smith Room), S60 2HB. This is an opportunity to provide an overview of what the Council is looking for and how to apply. If you are interested in attending the event on the 25th March, please email: Commissioningenquiries@rotherham.gov.uk Summary of the services required Services for adults with a Learning Disability and /or Autism which have been identified in a Support Plan and achieve outcomes that: - Improve quality of life and independence, through supporting people to be able to access health and social activities, - Help individuals gain essential life skills, improve life opportunities and potentially employment opportunities. - Provide short breaks opportunities for carers to support adults living at home for longer. Services may be delivered online, individually or within groups, as appropriate and subject to individual choice. The FPS will remain open to new applicants and providers can bid for one or multiple Lots: Lot 1 Community Opportunities Enablement Services (up to 6 months) Community opportunities under this category will be time limited with clear goals and outcomes around identifying and meeting personal aspirations, supporting the person to gain or regain skills (e.g., independent living skills, or skills that will enable them to get paid work), connecting the person with local groups, services and interests. Packages under this category will be for an initial period of up to 12 weeks. Lot 2 Community Opportunities Personalised Services (up to 12 months) Service delivery for People with more complex needs, and/or frailties with the aim of offering goal-oriented support to help them become more independent, feel more confident and maximise their abilities. Services may be offered for a longer period dependent on the person's needs and aspirations. Lot 3 Community Intensive Support Opportunities (Complex Needs) These services will replicate and expand on the current building based and outreach support for people with multiple and profound disabilities (Complex Needs). This Lot will cover services for people with learning disabilities and autistic people, but may also include sensory impairment, physical disability, epilepsy, dementia and additional mental ill health. Individuals with acquired brain injury may, where appropriate, be referred. Services are likely to be longer-term and will be offered to a cohort of people with more complex needs who may require personal and health care as well as activities to promote independence. Therefore, CQC registration may be applicable. This Lot includes an ambition to develop the market and explore innovative offers to provide support for Carers to continue in their caring role, and may include evening and overnight respite. Note: the existing Respite accommodation is not currently in scope for this procurement. Lot Z Future needs for Community Support This lot will include miscellaneous contracts relating to future needs of autistic people and people with learning disabilities. This may include specific specialist requirements for example Dementia, combined learning disabilities and autism.
